Survey ManagerTM - Windows version only

Insert the diskette labeled 1 in your floppy drive (A: or B:).
From your Windows program manager, click on File and then click on Run
At the Command Line prompt type A:\setup (or B:\setup) and press ENTER.
Follow the setup instructions which appear on the screen in the
setup window.

Double click on The Survey ManagerTM icon.
After The Survey ManagerTM has loaded, click OK in the title window.
NOTE: Press F1 key for context-sensitive help, or click the help button.

Click on File.
Click on New Survey and type a name (up to 8 characters) in the file name
text box.
Click OK.
A survey information window will appear. In the text box, type in a
more descriptive name for your survey (up to 50 characters). You can also
type in a detailed description of the survey and/or a brief set of
instructions for completing the survey (up to 250 characters).
Click OK when finished.

Click on Survey.
Click on Question Design.
Click on the Group name text box and type in a group name such as
administration, communications, etc.
Click on the Instructions text box (if desired) and type in
specific instructions for the question, or the group of questions.
Click on the Number text box and type in 1. Click on the Text text
box and type in the question.
Click on Scale window.
Click on the down arrow after the Scale text box and pull down list
will appear with the resident response scales.
Click on your scale choice (e.g. agree/disagree).
Click on the up and down arrows after the Number text box to choose
the response scale number (e.g. 5 for a 5-point agree/disagree scale).
Click on the down arrow after the Orientation text box.
Click on either Vertical or Horizontal orientation style, (i.e., how
the scale will be formatted on the survey form).
Click on View to see the survey as formatted by the built-in word processor.
Repeat steps 5-8 to enter your survey questions. Click on View once all
your questions are entered and use the formatting options to design a
custom survey form.

Click on File and Print to bring up the print window. Set desired settings and click OK to print the survey form to your printer.
NOTE: Only format your survey after you have entered all your questions. The Survey Manager'sTM word processor will not save formatting. Consider exporting the survey by clicking on Survey and then Export. This function will export the survey in an .rtf file which can be uploaded by any commercially available word processor.

Click on Survey and then Codes.
Click on the first line under Survey Codes and then type in
an alpha/numeric survey code (e.g., 001).
Press Tab key to move to the Description line and type in a description
of that code (e.g., Headquarters). Press Tab key again to enter a second
code/description and repeat steps until you have entered all your codes.
Click OK when finished.

Click on Data and Add/Edit.
Enter the respondent number (e.g., 1) in the text box and click OK.
Click on the down arrow after the Survey Code text box. Click on the code
which corresponds with the survey being entered into the program at this
time.
Click on (or use the numeric keypad to enter) the corresponding response(s)
for each question. After you are finished entering all responses, click
on Next if you want to enter another survey or OK if you want to save
the responses and exit the Add/Exit window.

Click on Reports.
Click on the report format and analysis options found within the
Report window (e.g., Percentages," Multiple Questions per Page, etc.).
Click on Survey Code."
The matrix that appears allows you to compare and combine codes using the
report options that follow. In column 1, click on the box for each code that
you want to combine together. An X will appear in those spaces. Within
column 2 (through 10), click on the space(s) for those codes that you
wish to compare. An X will appear in those spaces.
For example, you might choose to combine codes 001, 003, and 004 (X in
column 1 for each code) and compare this aggregate with code 005 (X in
column 2 for this code). You can combine an unlimited number of codes. You
are limited to comparing 10 codes simultaneously.
Click on Questions.
Click on the boxes to activate one or more of the options listed.
Click on Show Graph or Show Table when you have finished choosing your
options. The report will be presented on the screen.
Click on Print to print to your printer.

Click on File and Exit.
